    3. Use of Services

You agree to use our website, forms, communications, and services only for lawful purposes and in accordance with these Terms.

You agree not to use our website, forms, communications, or services in any way that violates any applicable law or regulation, interferes with the proper operation or security of our website or systems, impersonates or attempts to impersonate Mindara or another person or entity, transmits malicious code, spam, or harmful content, collects information from our website without authorization, or submits false, misleading, or incomplete information.

    4. Telehealth and Care Disclaimer

Information provided through our website, communications, or general informational resources is provided for informational purposes only and is not intended to replace individualized medical, psychiatric, or professional advice. Care needs, treatment eligibility, service availability, and appointment types may vary depending on each individual's circumstances and needs.

Submitting an inquiry, requesting a consultation, completing a website form, or communicating with Mindara does not create a patient-provider relationship unless and until the applicable intake, consent, and treatment agreements have been completed and accepted.

If you are experiencing a mental health emergency, call 911 or go to your nearest emergency room. You can also contact the 988 Suicide & Crisis Lifeline by calling or texting 988, or chatting at 988lifeline.org/chat.

    5. SMS Communications

Message frequency may vary based on your interactions with our services. Message and data rates may apply for messages sent to you from us and to us from you. If you have questions about your text messaging or data plan, please contact your wireless provider.

You can cancel SMS communications at any time by replying “STOP” to the phone number from which you received our messages. After you send “STOP,” we may send you a confirmation message that you have been unsubscribed. After this, you will no longer receive SMS messages from us unless you opt in again.

Carriers are not liable for delayed or undelivered messages.

Opting out of SMS communications does not affect other forms of communication unless separately requested.

Consent to receive SMS messages is not a condition of purchase, treatment, or use of our services.

    6. Eligibility

You must be at least 18 years of age to use our website or consent to receive SMS communications from us.

Individuals contacting Mindara on behalf of a prospective or current patient represent that they are authorized to provide the information they submit and, where applicable, to communicate regarding that individual's care.

Eligibility for specific services, appointment types, or accommodations may be subject to separate intake assessments, consent requirements, and applicable laws or regulations.

    7. Inquiries, Forms, and Communications

You agree that information you provide through our website, forms, scheduling tools, phone calls, SMS messages, emails, or other communications is accurate and complete to the best of your knowledge.

Submitting a contact form or requesting a consultation does not guarantee appointment availability, a particular provider, a specific level of care, or any particular outcome.

Mindara reserves the right to determine whether available services are appropriate for a prospective patient's needs, subject to applicable law.

    8. Pricing, Payments, and Fees

Prices, session rates, and insurance information are provided separately through our Cost & Insurance page and applicable treatment or billing agreements.
